Electronic Signature Authorization Form
The ESAF submission deadline for the Fall 2017 exams has passed.
If you submitted an ESAF before the deadline of August 21 but did not receive the confirmation email, please check your junk mail folder before contacting the office.
If you do not have an ESAF on file and would like to register for a Fall 2017 exam, please submit a printable registration form by mail (Casualty Actuarial Society, 4350 N. Fairfax Drive, Suite 250, Arlington, VA 22203), email (firstname.lastname@example.org), or fax (703.276.3108).
ESAFs received after August 21 will not be processed.
Who Needs to Submit an ESAF?
Use this flowchart to help you determine whether or not you need to submit an ESAF:
If you do not have an account, do not fill out a New Visitor Registration request. An account will be created for you using the information on your ESAF.
If you have an account and need assistance registering online or accessing the CAS website for any other purpose, please contact Sophie Uy for assistance.
|Print legibly.||Send multiple ESAFs.|
|Make sure that the form is dated and signed.||Sign your ESAF by typing your name on the signature line.|
|Sign your ESAF by hand.*||Include special characters (@, $, %, !, etc.) in your password.|
|Send your ESAF as an attachment if sent via email (.pdf or .jpg).|| Send a password-protected ESAF.
*Only hand-written signatures will be accepted. Digital or typed signatures will NOT be accepted; the form is treated as a legally binding document and must have a legal written signature.
ESAF Processing Timeline
Please allow at least two weeks for the form to be processed, except if a submission deadline date is posted. Forms received by the stated deadlines will be processed in time for that sitting. For each exam sitting, the ESAF submission deadline is approximately 7 to 10 days prior to the exam registration deadline. The ESAF submission deadline will be posted at the top of this page once it has been determined.
If submitting a form close to the deadline for exam registration, please follow up if you do not receive a confirmation at least one day prior to the deadline. The CAS Office may not be aware that you did not receive the confirmation or your form may not have been received. No exceptions are made if the exam registration deadline is missed.
You will receive an email confirmation once the form has been processed. Do not try to register online for exams if you just submitted an ESAF; please wait until you receive the confirmation email. Make sure you check your junk mail folder!
